Initial Appointment
- You will be asked to complete questionnaires about your symptoms.
- Please bring your insurance card to the first appointment.
- If you have worked with other health care providers in the past, you may be asked to sign a release of information form for each.
- Both parents should accompany a minor child to the first appointment.
- If special accommodations are required, please inform the receptionist prior to the appointment.
Future Appointments
- Call our office at (507) 282-1009
- Future appointments can be scheduled at the office or by calling during normal business hours 8am-5pm, Monday-Friday.
- The Rochester office also makes appointments for Owatonna.
Cancellation
- We request at least 24 hours notice for all cancellations or rescheduling.
- Bluestem Center reserves the right to bill some or all of our regular fee for missed appointments, including "no shows" not cancelled in advance.
Other Children
- If you need to bring other children along to the appointment, please arrange for another adult to watch them in the waiting room. This way, you and your child will be able to work with a staff member in privacy.
After Hours Service - Call (507) 282-1009
- We provide after hours telephone access for Bluestem Center patients.
- Between 5 PM and 8 AM, calls are forwarded to a staff member.
- Please leave a message, and we will return your call.
- If it is an emergency, call 911. We will follow up after you have received emergency care.
