- You will be asked to complete questionnaires about your symptoms
- Please bring your insurance card to the first appointment
- If you have worked with other health care providers in the past, you may be asked to sign a release of information form for each
- If the patient is a minor child, both parents should attend the Triage appointment without their child
- If special accommodations are required, please inform the receptionist prior to the appointment
